We’re looking for a Physical Therapist to take on a part-time Mobile Physical Therapist (Home Health) role serving Southern and Southwest Minnesota. • Competitive per-visit pay ranging from $50–$100+ depending on visit type.
  • Sign-on bonus: Up to $8,000 for full-time / $4,000 for part-time.
  • The ability to make a measurable impact on patient outcomes.
  • A role where driving is part of the adventure—not a burden.
  • A supportive team rooted in 30+ years of high-quality community care.

Benefits:

  • Sign-on bonus: $8,000 full-time or $4,000 part-time.
  • Mileage reimbursement at $0.65 per mile between clients.
  • Full-time benefits (32+ hours): medical, dental, vision, PTO, 401(k), life insurance, short-term disability, long-term disability.

Location Highlights

Explore Southern and Southwest Minnesota from a base in Blooming Prairie. The region offers small-town character and community events, access to outdoor recreation including fishing and boating on area lakes, and nearby parks and trails for hiking and cycling. Local towns provide dining options, farmers markets, and seasonal festivals. Mankato and North Mankato are nearby for shopping, cultural venues, and regional healthcare resources. The position includes occasional in-person meetings and orientation at the main office in Golden Valley, MN.

Impacting Patient Care Nationwide
