Job Title: Physical Therapist - Full Time (#441911)


Physical Therapist — Skilled Nursing Facility. Days.


We are seeking a Physical Therapist for a full-time, permanent role at a skilled nursing facility in Pleasant Prairie, WI. The clinician will evaluate, plan, and implement therapeutic interventions to restore or improve patient mobility and function. The position supports patients recovering from acute injuries and those managing chronic conditions. The schedule is Days, 5x8, 0830-1630.


Minimum Requirements:

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Wisconsin state PT license required to start the position.
  • BLS certification from American Heart Association or American Red Cross may be required.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Demonstrated competency with restorative therapy programs and mobility assessment.
  • Ability to document patient care in electronic medical records in a timely manner.
