Job Title: Physical Therapist - Full Time (#463095)


Physical Therapist — Home Health. Fort Dodge, IA. Days.


We are seeking a Physical Therapist to join a home health team in Fort Dodge, IA. The clinician will provide in-home assessment, skilled therapy interventions, and patient/caregiver education to promote functional recovery and safe independence. This is a full-time, direct-hire opportunity with daytime hours Monday–Friday, 8:00am–4:30pm.


Minimum Requirements:

  • Graduate of a physical therapist education program accredited by CAPTE, or equivalent education determined by an approved credential evaluation organization.
  • Passed a state-approved examination for Physical Therapists.
  • Active PT license in the state of practice. Compact licensure accepted.
  • Minimum one year of experience caring for a patient population similar to those served in home health.
  • Ability to perform physical therapy assessment and treatment in the home setting without direct supervision.
  • Reliable transportation and ability to drive safely in varied conditions.
  • Basic computer skills, including Microsoft Office.
  • Strong interpersonal and teamwork skills. Ability to understand and apply policies and procedures.
  • BLS certification may be required.


Preferred Qualifications and Experience:

  • Knowledge of commonly used medications, modes of administration, actions and side effects.
  • Experience with OASIS data collection and documentation.
  • Experience supervising home health aides or PT assistants.


Position Overview and Responsibilities:

  • Provide initial and comprehensive in-home assessments and ongoing interdisciplinary evaluations.
  • Develop, implement, and update measurable home health plans of care and discharge plans in collaboration with patients, families and physicians.
  • Deliver therapeutic interventions and modalities consistent with physician orders and clinical best practices.
  • Educate patients and caregivers on treatment, disease management, medication use, equipment safety, and emergency preparedness.
  • Maintain accurate, timely clinical documentation and time/travel entries in accordance with home health CoPs, payer requirements and accreditation standards.
  • Obtain and document physician orders and communicate changes in patient condition to the physician and care team.
  • Identify and intervene to reduce risks for hospitalization or emergency department use.


Additional Client Requirements:

  • Candidate must not have applied directly to the position when submitted by an agency.
  • Support for home care visit documentation and adherence to CMS guidance is expected.
