Minimum Requirements
- Associate, Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
- Active Kentucky Physical Therapy license required to start the assignment.
- Home health experience required. Experience with OASIS assessments and documentation using HCHB required.
- Experience performing admissions, recertifications, 30-day assessments, and discharges in a home health environment.
- Experience supervising PTAs or coordinating care with PT assistants required.
- BLS certification from AHA or ARC may be required.
- Minimum education requirement: Associate degree (client requirement).
- Preferred: prior travel or remote caseload management experience. Preferred: familiarity with mileage reimbursement processes between visits.
Assignment Details
- Facility type: Home Health.
- Shift: Days.
- Schedule: Monday through Friday, typical daytime hours (client indicates Mon–Fri am to 4:30 pm).
- Weekly guaranteed hours: 32 hours per week.
- Productivity expectation: 30 points.
- Coverage area: Logan and Todd counties. Mileage reimbursed between first and last patient visits.
- On-call/weekend requirement: No.
- Dress code: royal blue scrubs.
- Floating: Candidate may be expected to cover patients across the specified coverage areas.
