Full Description

Job Title: Physical Therapist - Travel Contract (#445548)

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy in a home health setting for patients across the lifespan in West Plains, MO. The Physical Therapist plans, implements and evaluates individualized plans of care based on medical referral and clinical assessment. Collaborate with interdisciplinary team members to restore or maintain patient function and independence. Educate patients and families on therapeutic procedures, safe use of durable medical equipment, and home transfer techniques. Document evaluations, Plans of Care, progress notes and other clinical records in a timely manner. Follow infection control and universal precautions. Travel locally and perform physical tasks including lifting, bending, standing and walking as required.


Minimum Qualifications

  • Current state license as a Physical Therapist required and active in Missouri prior to the start of the assignment.
  • Bachelor of Science, Master of Science, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA required.
  • Proof of current CPR certification required.
  • Current driver’s license, reliable transportation and vehicle insurance required.
  • 1-2 years of experience as a Physical Therapist in acute care or rehabilitation setting required.
  • Ability to perform physical job demands including heavy lifting, prolonged standing and walking.
  • Must read, write and speak fluent English.


Preferred Qualifications

  • Previous home health experience preferred.


Assignment Details

  • Ability to work a flexible schedule and travel locally.
