Job Title: Physical Therapist - Travel Contract (#447499)
Looking for a change of scenery and a chance to help others. Join a traveling Physical Therapist team in New Braunfels, TX, working in home health where you will help patients recover from injuries and manage chronic conditions while providing skilled care in patients' homes. This role requires professional clinical decision-making, documentation, and coordination with interdisciplinary teams to support patient independence and safe discharge planning.
Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active Texas Physical Therapist license required to start the assignment.
- Professional certification required. Bachelor-level degree minimum as noted in order details.
- BLS certification from AHA or ARC may be required.
- Home health experience required or strongly preferred, including delivering skilled care in patients' homes.
- Experience with electronic health record documentation; familiarity with HCHB is preferred.
Preferred Qualifications
- Prior home health physical therapy experience.
- Experience with HCHB (Homecare Homebase) electronic documentation.
Assignment Details
- Shift hours: 2000-0400.
- Weekly hour guarantee: 36 hours.
- Coverage area up to a 60 mile radius from base caseload.
- On call/weekend requirement: No.
- Mileage reimbursed between first and last patient visit.

Impacting Patient Care
Provide patient-centered skilled physical therapy in home settings to support recovery, mobility, and safety. Perform comprehensive evaluations, create individualized treatment plans, and communicate progress with patients, families, and interdisciplinary teams. Documentation and adherence to home health regulatory and billing guidelines are required. This role supports community-based care and continuity of services across the post-acute continuum.
