Job Title: Physical Therapist - Travel Contract (#447505)
Looking for a change of scenery and a chance to help others. Provide in-home skilled physical therapy services to patients in St. Petersburg, FL, helping them recover from injuries and manage chronic conditions while working in a home health setting. Evaluate patients, develop and implement individualized plans of care, deliver therapeutic interventions to improve mobility, strength, balance, and safety, and educate patients and caregivers on disease management and home exercise programs. Ensure accurate, timely, and compliant documentation supporting medical necessity and homebound status. Collaborate with physicians and interdisciplinary team members, communicate changes in condition, participate in care coordination, complete OASIS, adhere to agency policies, and supervise PTA services when applicable.
Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
- Active Florida PT license required to start the assignment.
- Home health experience required.
- OASIS knowledge required.
- Experience with HCHB electronic documentation required.
- BLS certification from AHA or ARC may be required.
- Minimum education: Bachelor's degree.
Preferred Qualifications
- Previous traveling physical therapy experience preferred.
- Experience supervising PTA services preferred.
Assignment Details
- Shift start and end times: 0800 - 1600 (M-F schedule noted as 8:00 AM–4:30 PM in order notes).
- Weekly hour guarantee: 40 hours.
- On-call/weekend requirement: N/A.
- Mileage reimbursed between visits, from first patient to last patient: Yes.
- Coverage area: St. Petersburg area.
- Candidates may be required to follow agency policies including dress code (any color scrubs per agency policy).
