Job Title: Physical Therapist - Travel Contract (#463128)

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CPR certification required
• Doctor of Physical Therapy required
• Home Health experience required

Assignment Details

• Shift hours: 8:00 AM - 4:30 PM
• Coverage area: 50-mile radius
• Mileage reimbursed between visits, from first patient to last patient
• Responsibilities include SOC, evaluations, ROC, recerts, and subsequent visits
• Dress code: Business casual
