Job Title: Physical Therapist - Travel Contract (#463808)

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• CA license required
• BCLS required
• EMR: HCHB required
• 1 year of recent Skilled Home Health experience required, including at least 1 year within the last 3 years
• Oasis experience is required
• Traveler is expected to have their own transportation

Assignment Details

• Monday through Friday, 8:00 AM to 5:00 PM
• Patient population: Geriatric
• Occasional travel radius of 50+ miles
• May visit up to 6 patients per day
• 30 productivity points standard
• Ratios: 1:1
