Job Title: Physical Therapist - Travel Contract (#463848)

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• At least 1 year home health experience
• HCHB experience
• Current/Active Physical Therapy License

Assignment Details

• Monday through Friday schedule
• 8 hours per day, 5 days per week
• Shift hours: 8:00 AM - 4:30 PM
• No on-call or weekend requirement
• Coverage area includes Highlands and Hardee Counties
• Mileage reimbursed between visits, from first patient to last patient
• Provide Physical Therapy services in patient homes, including OASIS visits
• Productivity expectation: 30 points
